Healthy Travel
You will be traveling soon and, of course, you would like to return home healthy. Staying abroad can pose a risk to your health. Fortunately, you can minimize some of the health risks.
I recommend getting tailored travel advice from HACUSERK at least 6 weeks before your departure. Better late than never applies when traveling “last minute.
Even if you are not a patient in our practice, you are welcome at HACUSERK.
With me, you get tailored advice. I take into account your specific health condition, travel purpose, duration of travel, itinerary and mode of travel. I am a specialized travel medicine general practitioner who can give you expert advice on this and I work according to the guidelines of the National Coordination Center for Travel Advice(LCR).
Intake form
You can pick up a “traveler intake form” at the practice, but you can also fill out the form online by clicking on the link.
(For travel advice, it is important to know if you were vaccinated against measles (BMR) as a child or had measles. You can indicate this on the form).
After we receive the form from you, I will then contact you, to set up an appointment. At the consultation, advice is discussed and any necessary vaccinations administered. Do you already have an international vaccination certificate/the yellow booklet? Then bring it with you to the appointment.
Cost
The cost of the consultation is €55.50 per person. This amount is a national rate which is in principle reimbursed if you have supplementary insurance. Check with your insurance company.
This amount is for: compiling the advice, administering the vaccinations, an international vaccination certificate and information leaflets. The cost of vaccinations will be billed to you separately by the pharmacy. Thus, you will receive two bills to submit to your health insurance company.
